Medical Records Issuance

Notice
In order to protect patient’s personal information, the medical institution is responsible for keeping the confidentiality of the medical records.
The patient’s medical records can be accessed and issued only with the patient’s informed consent.
[Related law – Medical Service Act Article 19 (Prohibition against Disclosure of Confidential Information) & Article 21 (Inspection of Records, etc.)]

Required documents

The documents required to issue a copy of the medical record below are divided into categories, types and required documents.

Categories Type Required Documents
Patient Patient Patient’s ID
(ID card, passport, driver’s license, or other IDs issued by public organization)
Guardian Spouse, patient’s direct ancestor or descendant, or spouse’s direct ancestor or descendant 1. Applicant’s ID card
2. Certificate of family relations (Copy of resident registration)
3. Consent form signed by patient (except for patient under 14 years of age)
4. Copy of patient’s ID card (except for patient under 17 years of age)
When patient has died 1. Applicant’s ID card
2. Certificate of family relation (Copy of resident registration)
3. Document proving the death of patient, such as certificate of family relation, copy of resident registration, or death certificate
When patient is unconscious or unable to sign due to severe illness or injury 1. Applicant’s ID card
2. Certificate of family relations (Copy of resident registration)
3. Medical certificate stating patient’s disability to sign due to severe illness or injury
When patient is missing 1. Applicant’s ID card
2. Certificate of family relation (Copy of resident registration)
3. Document that approve the fact of disappearance, such as copy of resident registration or copy of official document from the court
When patient is mentally incapable 1. Applicant’s ID card
2. Certificate of family relation (Copy of resident registration)
3. Copy of patient’s incompetency sentence document from the court or medical certificate approved by psychiatrist
Authorized
Representative
The third party, such as patient’s sibling or insurance company 1. Applicant (representative)’s ID card
2. Consent & Authorization form signed by patient
(But, in case of patient under 14 years of age, legal representatives fill in the form and the document such as certificate of family relation should be attached.)
3. Copy of patient ID card (But, except for the patient under 17 years of age)

Request of Medical Records Copy

Outpatient

  • Patient without clinic appointment
    1. Medical Records Copy Desk (No.12) : Request medical records/Submit required documents
    2. Payment·Registration : Pay the fee for medical records and image copy
    3. Medical Records Copy Desk (No.12) : Receive the requested medical records (※In case of request at outpatient clinic, please submit required documents.)
  • Patient with clinic appointment
    1. Outpatient Clinic : Consultation with doctor/Request medical records
    2. Payment·Registration : Pay the fee for medical records and image copy
    3. Medical Records Copy Desk (No.12) : Receive the requested medical records (※In case of request at outpatient clinic, please submit required documents.)


※ In case of psychiatry department, medical records are issued with doctor’s approval after registration.
 

Inpatient

  1. Step 1 : Request medical records at ward nurse station
  2. Step 2 : Consultation with assigned doctor
  3. Step 3 : Fill out the form and receive at Medical Records Copy Desk (No.12)

Copying medical records (Pages 6+ ) KRW 100 per page
Image copy (CD) KRW 10,000
